How We Evaluate Low Back Pain

Effective treatment begins with determining why the pain is occurring. Your evaluation may include a detailed history, orthopedic and neurologic examination, spinal range-of-motion assessment, muscle strength, reflex and sensory testing when indicated, and examination of spinal joint and soft-tissue function.

Prior X-rays, MRI studies, and other diagnostic tests are reviewed when available. Additional imaging may be recommended when the examination or clinical history indicates that further investigation is appropriate.

When MRI May Be Appropriate

Most uncomplicated low back pain does not require an MRI. However, advanced imaging may be appropriate when symptoms or examination findings suggest disc herniation, nerve-root involvement, significant trauma, progressive neurologic findings, or when symptoms fail to improve as expected. When MRI is indicated, the images are correlated with the patient’s history and examination findings to determine their clinical significance.

Disc herniation, foraminal narrowing, or other spinal conditions may produce symptoms that extend beyond the lower back and into the buttock, thigh, leg, or foot.

When these symptoms are present, a more detailed neurological and spinal evaluation may be appropriate to determine whether additional diagnostic testing or imaging is necessary.
